Amendments can be categorized under three groups

Reasons for amending member names

 
 

1.

Discrepancies in the initials of a member name

 
   

2.

Addition or deletion of initials of the name

 
   

3.

The initials are not in the correct order

 
   

4.

Discrepancies of the names denoted by initials

 
   

5.

Another member’s name has been mistakenly used for the correct name

 
   

6.

Use of new names instead of the name given in the birth certificate

 
   

7.

Change of name after marriage

 
   

8.

Use of false names or nicknames

 
   

9.

Payment of contributions for many members under a same number

 
       
 

Amending member names

 
 

The following steps should be taken depending on the nature of the amendment:

 
 

1.

A letter from the employer confirming the old or incorrect name and the new or correct name with a proof of identity should be sent to the Superintendent, EPF, Central Bank, Colombo 1 or to the Labour Commissioner. This is sufficient in the case of a discrepancy in the initials of a name.

 
   

2.

If the previous employer is no longer active, a letter from the GS of your area, conforming your name, certified by the District Secretary and confirmed by the Deputy Labour Commissioner / Labour Officer, should be sent. Attaching the B card is very important.

 
   

3.

In the case of a total name change, all contribution details should be sent along with the letter of request.

 
   

4.

For changes due to marriage, or change in birth certificate, the copies of all birth/ marriage certificates and legal documents certified by the employer should be sent. In the event of the previous employer no longer being active, a letter from the GS of your area, confirming your name, certified by the District Secretary and confirmed by the Deputy Labour Commissioner / Labour Officer, should be sent. Attaching the B card is very important.

 
       
 

Amending the NIC number

 
 

A letter from the employer confirming the name, old NIC number, new NIC number and the reason for change, should be sent. In addition to this, where necessary a written confirmation from the GS or registrar of persons is required. Since the change of NIC number is critical, proper documentation should be sent along with the request.

 
 

Why amend the NIC number?

When proving account balances

 
    The NIC number held at EPF and the NIC number submitted by the member should match.
